If you have a confirmed appointment but haven't received your account credentials, use the link below to request account activation. Our team will ensure your account is activated within a few hours.

Enter your username and password in the fields provided.

Click the "Sign In" button to access your account.

If you do not have an account yet:

Click on the button below " Enable my account " to request your account be enabled.

Please type in your First Name, Last Name and Date of Birth.

Send the request.

Wait for the healthcare provider to send your account credentials to your email address, which will include a username and password.

Once you receive your account credentials, follow the steps above to sign in.

Download and fill out the necessary forms provided below.
Save the completed documents on your device.
Return to this webpage and click the "Email Completed Paperwork" button to upload and submit the documents.

Please make sure to add your full name and date of birth.
