Patient Portal

 IMPORTANT: 


If you encounter difficulties accessing the patient portal for submitting the paperwork, use the button below to manually submit the documents.

For new clients:

1.  Wait for our team to confirm your appointment.
2.  Once your appointment is confirmed, we will send you your account credentials via email.
3.  If you have not received your account credentials and your appointment has been confirmed, call us at the numbers below or click on the button to send a follow-up email.

For existing clients with sign-in credentials:


Here are the steps to access the patient portal for existing clients with sign-in credentials:

1.  Click on the button below " Patient Portal " to go to the sign-in page for the patient portal.
2.  Enter your username and password in the fields provided.
3.  Click the "Sign In" button to access your account. 

If you do not have an account yet:

1.  Click on the button below " Enable my account "  to request your account be enabled.
2. Please type in your First Name, Last Name and  Date of Birth. 

3. Send the email. 

4. Wait for the healthcare provider to send your account credentials, which will include a username and password.
5.  Once you receive your account credentials, follow the steps above to sign in.
